About your First Visit

At your first visit, we ask you to arrive a few minutes early to complete the required patient information, health history, and privacy documents. Dr. Mitschke will perform a comprehensive dental health examination that may include radiographs to check for bone health and decay, periodontal charting to check gum health and an oral cancer screening. Dr. Mitschke will then recommend treatment options to obtain your ultimate smile. The office staff will outline a treatment plan and payment arrangements to ensure affordable terms for your dental care.
To make your first visit as convenient as possible, please bring the following items:
- Completed and signed new patient forms
- Dental health insurance information, if applicable
- Driver’s license or other form of identification
- List of current medications, including vitamins and supplements
- Form of payment (cash, check, or credit card)
